If you have any questions, please feel free to call the office at your convenience.  Many of your questions can be answered by our office staff.  When it is necessary to contact us during non-scheduled office hours, our 24-hour answering service will handle your request and we will return your call as soon as possible.  If you feel an emergency exists, advise the answering service accordingly and ask to have the doctor pages immediately.

We consider quality care an investment in your health.  It is our objective to provide you with the best available care at a reasonable cost.  In an effort to eliminate the expense of billing, you can help by making your payment at the time of your visit.  You may pay by check, cash or any major credit card and you will recieve a reciept for your records.

Scheduling of appointments can be made any day of the week with the receptionist or the answering service.  Be sure that you clearly specify which office location (5511 1/2 W. Montrose or 150 E. Huron, Suite  801 ) you wish to be seen so that it can be correctly scheduled.  Of course, emergencies can always be handled as soon as possible by calling either office.  We are aware many of our patients are busy and require a specific time slot.  For this situation, we suggest that an appointment be made upon leaving the office after each visit.

To simplify filing for covered services with your insurance carrier, we will issue you an itemized reciept upon payment of your bill and/or help you complete your insurance forms.  We can also submit claims and accept assignment of benefilts for the various HMO and PPO plans that we are contracted with.  Please note: if we accept assignment you then are responsible for any deductibles or co-payments.  We must stress, however, that the ultimate financial responsibility for services rendered rests totally with you.
